Before downloading the application form, please be sure to read the Age and Attendance requirements (also available under the “Requirements” item in the menu at the top of this page). You should also download and review the Program Summary to make sure you will be able to commit to the requirements of the program.

Please click on the link at the bottom of this page to download the program application. Once you have downloaded it, please follow these steps:

• Print the application form (total of 3 pages).
• Read the instructions carefully and complete the form.
• Mail it to the address listed on the form.

IMPORTANT: Your application form must be RECEIVED by October 31, 2019. Please plan ahead and mail it with plenty of time to spare!
